Job Title: Licensed Practical Nurse (LPN)
Department: Wellness
Category: Hourly
Reports to: Director of Wellness/Assistant Director of Wellness

General Purpose:
Provides general oversight to residents’ care. Provides supervision of caregivers and med-techs. Assesses residents’ condition and need for treatment. Monitor and provides medication administration and nursing care treatments. Reports to and supervised by the Director of Wellness, Assistant Director of Wellness.

Responsibilities:
– Contacts physicians, family members and other medical resources as necessary to ensure care is provided when needed.
– Documents all pertinent information in resident medical record.
– Transcribes physician’s order and follows through with those orders.
– Monitors and administers medications to residents per physician’s orders.
– Assesses resident’s condition during move in and throughout occupancy.
– Provides hands-on care to residents as needed.
– Schedules resident appointments.
– Reports all incidents to the Director of Wellness and documents according to company policy.
– Reviews resident records, documents, enters and updates orders in resident management system (Alis, currently) as needed.
– Supervise caregivers and med-techs.
– Document incidents involving residents, including their care and continued health care needed.
– Performs other duties as needed.

Skills and Qualifications:
– Must have current State of Florida Licensed Practical Nurse license.
– Must complete the continued competency requirements for renewal of an active Licensed
Practical Nurse license according to the State of Florida and maintain original documentation of completion.
– Demonstrate ability to communicate effectively in English, both verbally and in writing.
– Maintain working knowledge of all policies and procedures pertaining to the position.
– Meet state or locate health related requirements.
– Maintain any training as required by state or local regulations.
– Must be considerate and polite to others, possess strong customer service skills and able to accept supervision, training and direction.
– Must possess the ability to function respectfully in busy, sometimes chaotic, situations.
– Must possess strong customer service skills.
– Must always maintain professional appearance with appropriate attire, grooming and hygiene.

Physical Requirements:
– Must be in good physical and mental health, capable of handling the job duties and
responsibilities.
– Must be able to walk and stand for an extended period.
– Must be able to exert up to 75 lbs. of force occasionally and exert up to 50 lbs. of force frequently to lift, carry, push, pull or otherwise move objects, including the human body.
